Why Period Cramps Are So Painful

What Is Period Pain?

Period pain differs from PMS (premenstrual syndrome), which often includes bloating, irritability, and fatigue before your period starts. Instead, dysmenorrhea refers specifically to the pain and cramping that occurs typically during menstruation, caused by the uterus contracting to shed its lining.

Types Of Period Pain

Primary Dysmenorrhea
Primary dysmenorrhea is the common cramping that often hits right before or during your period. This pain happens because of natural chemicals called prostaglandins, produced in the lining of the uterus, which cause the muscles and blood vessels there to tighten.

Here’s a closer look at why cramps can feel so intense:

  1. Uterine Contractions: Just like labour contractions, the uterus tightens to help shed its lining. For some, these contractions can be quite powerful and feel similar to muscle cramps.

  2. Restricted Blood Flow: As the uterus contracts, blood vessels around it constrict, which can briefly cut off oxygen to nearby muscles. Lack of oxygen makes the cramping feel sharper and more intense, contributing to that throbbing sensation.

  3. Inflammatory Response: Prostaglandins don’t just cause contractions; they’re also inflammatory chemicals. High levels can trigger a pain response similar to that of an inflammatory injury, creating additional discomfort.

  4. Nerve Sensitivity: During menstruation, nerves in the pelvic area may be more sensitive. This can make the cramps feel even more painful, as the nervous system is more alert to the sensation of the uterus contracting.

On the first day of your period, prostaglandin levels are usually at their peak, which can make the pain feel intense. As your period progresses and the uterine lining begins to shed, prostaglandin levels gradually drop, which is why the pain often eases after the first few days.

This type of pain usually starts when you first begin menstruating and often eases with age or after having kids.

Secondary Dysmenorrhea

Pain caused by conditions like endometriosis or fibroids. It often starts later in life and can worsen over time, persisting beyond your period.

When To Seek Medical Advice

Some pain is par for the menstrual course, but consider checking in with your doc if:

  • Cramps get worse or are suddenly unbearable
